We recommend that you inquire with your insurance provider to find out if a psycho-educational evaluation is a covered benefit of your policy and to determine whether a referral or pre-certification from your child’s primary care physician is required. Tell them you are seeking standardized cognitive performance testing, CPT code# 96125. This evaluation falls under medical benefits and not psychological or behavioral health benefits. Using the correct language when determining insurance coverage has been reported to make a significant difference in determining coverage and thereby reducing your costs associated with this type of testing. If your insurance provider does not cover testing, it is advisable to ask your tester if they operate on a sliding-scale payment plan.
